Hanley Foundation, a West Palm Beach-based non-profit
organization that aims to eliminate addiction through prevention,
treatment, advocacy, and recovery support, seeks a dedicated and
compassionate Case Manager for our Executive Program, Headwaters.
The Case Manager's primary objective is to prepare patients to
maintain recovery post-discharge and develop a care plan that meets
the patient's needs and lifestyle. In this role, you will
coordinate and facilitate patient continuing care in collaboration
with clinical teams. You will develop a written care plan for each
patient assigned to you at Hanley Foundation in cooperation with
the Patient, the Patient's Family, the Primary Therapist, and the
Referent. As a Case Manager, you will be a vital team member,
providing personalized care and support for our patients. This
position is scheduled for Monday through Friday, 8:00 AM to 5:00

Responsibilities:
- Assess, plan, link, monitor, and advocate for the most
appropriate services for each client.
- Learn and utilize Evidence-Based Practices.
- Provide case management services as needed for each
client.
- Complete and submit required documentation within specific time
frames.
- Attend monthly supervision.
- Attend mandatory training and meetings.
- Available to accommodate clients' schedules, including
evenings/weekends.
- Serves as a liaison for the client, family, referral source,
and other social service agencies.
- Facilitate timely communication regarding discharge placement
between patients, their families, the professional relations team,
the referent, and the referring facility.
- Coordinate all outbound travel arrangements.
- Coordinate and work with the multidisciplinary team daily to
effectively determine the best possible patient care.
- Advocate for our patients, guiding families to the right
resources and solutions in the community with integrity, knowledge,
and compassion.
- Maintain working knowledge of Medicare, Medicaid, and private
insurance company coverage for referred products and services.
- Participate in and complete discharge assessments, complete
follow-up phone calls in a timely manner, and provide referrals/
data according to the patient's needs.
- Represent Hanley Foundation by coordinating potential referrals
with the Admissions Department.
- Make follow-up calls 24 hours after discharge (including on
weekends).
- Document discharge discussions.
- Make and document Alumni calls (ongoing).
- Generate and accept calls from Alumni who may need support and
assist them by connecting them with internal and external
resources, as appropriate.
- Work collaboratively with the professional relations officers
and clinical team.
- Be responsible for protecting patients' health, safety, and
welfare.
- Ensure the privacy and confidentiality of all patients and
their records.
- Complete duties in a professional and timely manner.
- Report all alleged incidents of abuse or neglect following all
state standards, accrediting bodies, and Hanley Policies &
Procedures.
- Ensure patient records/documentation is accurate and prepared
promptly.
- Uphold all Hanley Foundation Policies and Procedures.
- Other duties as assigned.Knowledge/Skills/Abilities:
- Ability to recognize and understand a senior population's
physical, functional, social, and emotional needs.
- Ability to network and interact with the recovery community at
a local, state, national, and international level.
- Ability to work independently, exercise creativity, be
attentive to detail, and maintain a positive attitude.
- Must be computer literate and be familiar with most Microsoft
products and network components.
- Must have fluid communication skills that foster teamwork and
high morale.
- Must possess a high degree of organization skills and be able
to pass this information to his/her direct supervisor.
- Ability to maintain confidentiality of all medical, financial,
and legal information.
- Must be thoroughly familiar with and be able to demonstrate
knowledge of the principles and programs of the various 12-step
fellowships.Education/Experience/Qualifications:
- Bachelor's degree in healing arts preferred.
- Previous experience working in a treatment facility is
required.
- A sound knowledge of the teaching of the 12 Steps of Alcoholics
Anonymous is preferred.
- If in recovery, one (1) year of continuous sobriety is
